Physician's participation with an insurance plan is an individual
choice made by each Boone Clinic Physician. Their choice in
participation will effect the handling of your claim, any out-of-pocket costs,
and your billing by
Boone Clinic.
Please ask before your visit about
a physician's insurance plan participation status.
Statements that you receive
from your physician will include services provided for both you and your
family members. All statement balances should be paid within 30 days
of your receipt of the bill. We accept Master Card and Visa.
