If you
are seeing a physician who does not participate with your insurance plan,
or if you do not have insurance coverage, you will be responsible for
payment at time of service for all charges. If you have provided us
your current insurance information we will file your claim to your carrier
as a courtesy. Any payment made by your insurance carrier should be
received by you at your home address.
You may

arrangements to pay for your services or with any billing questions.
